The Health Assessment and Planning Partnership quarterly meetings are designed to:
• Foster networking opportunities • Provide feedback on the community health assessment and planning process • Hear updates on the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) Agendas will be sent out prior to the meeting.
